If you are considering moving to District 181, you will need to provide
information during the enrollment process:
- Health and cumulative records from your child's current school.
Parents will be asked to sign an "Authorization to Release Records"
form during the enrollment process
- Original Birth Certificate
- Proof of Residency (utility bill - gas, electric or water addressed
to you at your District 181 address; signed property/housing contract
or lease)
- If the student is transferring from a school within Illinois, parents
should bring a "Student Transfer Form" from the school from which
their child is transferring
- A new physical examination completed within six weeks of entering
a District 181 school for students transferring to District 181 from
out of state or out of the U.S. If the student has had a physical
examination within the last year and the examining physician is willing
to fill out the Illinois physical examination form, that also will
be acceptable.
- Whenever possible, parents should provide a copy of a transferring
student's immunization records on their first day of enrollment.
- Incoming kindergarten and fifth grade students will be required
to submit an Illinois physical examination form completed within the
last 12 months.
- Fifth grade students are required to complete the Hepatitis B vaccine
series.
